In connection with the preliminary list of potential agrarian reform beneficiaries (ARBs) for the landholding owned by

_________________________________________, which was posted at _______________________________________(place), on
___________________________ (date), I/we, the undersigned tenant/s/lessee/s/regular farmworker/s/ seasonal
farmworker/s/other farmworkers/actual tiller/s/occupant/s in the said landholding signify my/our interest and request/s that
my/our names be included as potential beneficiaries.
As petitioners, I/we declare that I/we possess the following basic qualifications of an ARB:
Landless as defined by R.A. No. 6657, as amended;
Filipino citizen;
Permanent resident of the barangay and/or municipality where the landholding is located.
Attached are the pertinent documents as proof of my/our qualifications as bonafide beneficiary/ies in the above landholding:
NOTE: Bring as many documents as possible and preferably, bring photocopies and the original document for comparison. The
MARO will retain only the photocopies after comparing with the original.
A.

FOR TENANTS, LESSEES, ACTUAL TILLERS/OCCUPANTS IN LANDHOLDINGS FOR INDIVIDUAL AND COLLECTIVE DISTRIBUTION TO ARBS (Please
Check Appropriate Box):
Community Tax Certificate (cedula) or barangay certificate of potential ARB as permanent or bonafide resident
of the barangay;
Voter’s identification card or certified copy of voter’s registration record;
Leasehold contract and/or proof of tenancy papers;
Certificate of Aggregate Landholding from the Municipal or City Assessor, land titles, tax declarations; and
Other documents that can establish the qualifications of the potential beneficiary

B.

FOR FARMWORKERS IN COMMERCIAL FARMS, PLANTATIONS AND OTHER LANDS QUALIFIED FOR COLLECTIVE DISTRIBUTION (Please
Check Appropriate Box):
Employment certificate;
Social security System (SSS) identification card;
Payslip/s or payroll;

Original or certified copy of decision/order or ruling by a court, quasi judicial body or administrative agency if
there was a case related to the dismissal, retrenchment, etc., of the potential ARB;
Original or certified copy of Notice of Dismissal or retrenchment for farmworkers beneficiaries; and
Original or certified copy of letter of resignation for farmworker beneficiaries.
