SIR:
The undersigned, respectfully applies for a [ ] License to Buy [ ] Permit
to Purchase/Re-dry/Process [ ] virginia [ ] burley [ ] native / dark,
leaf tobacco, for trading year ______, pursuant to Executive Order No. 245
and the Revised Tobacco Trading Rules and Regulations, and, that, he/she
had read and understood the provisions of the said Executive Order No.
245 and the Trading Rules and Regulations for Locally-Grown Leaf
Tobacco. Furthermore, he/she agrees that all documents/statements and
data given and attached to this application shall form part of the License to
Buy, Permit to Purchase / Redry / Process, that maybe issued in his/her
favor. Any false statement or misrepresentation, which may alter or affect
substantially the validity of this application, shall be sufficient ground for the
non-issuance/cancellation of the said License to Buy/Permit to Purchase /
Re-dry / Process.
