that the documents/statements and data given which are attached to
this application shall form part of the Permit/Authority/Accreditation
to Export/Import/Transship/Manufacture Tobacco that maybe issued
in our favor. Attached herewith are the documents required as stated
in the herein accompanying checklist.
I hereby affirm and confirm that all the statements and information
provided herein are true and correct on my own personal knowledge
and any false statement or misrepresentation, which may alter or
affect substantially the validity of this application, shall be sufficient
ground for the non-issuance/revocation of the said Permit/Authority/
Accreditation to Export/Import/Transship/Manufacture tobacco and
tobacco products.
