Where to File:

nearest SSS branch (for separated members forms must be submitted at the SSS branch where her previous employer's record is active)

How to File:

If you are a self-employed or voluntary member, you must submit Form MAT-1 and MAT-2 at the SSS branch where your record is active. Separated members must submit the forms at the SSS branch where her previous employer's record is active.

However, you may file your claim directly to the nearest SSS branch, although processing will still be done at the respective branch where your record or your previous employer's record is active.

Refer to page 2 of Form MAT-2 for the list of supporting documents that must be submitted together with the form.
