The Contributions Payment Return (Form R-5) is used by any employer-member to remit his employees' or household helper's monthly contributions.

You must fill up Form R-5 using black ink and submit it in four copies to any SSS-accredited bank or to the Cashiering Department at the SSS Head Office at East Avenue, Diliman, Quezon City, together with your payment.
